CVS Health Corporation is seeking a licensed Behavioral Health clinician to serve as a Clinical Case Manager. This 100% remote role supports members with complex mental health and substance use needs, coordinating care across providers and services.

The successful candidate will conduct assessments, develop care plans, and utilize Motivational Interviewing in a flexible, Monday–Friday schedule with no weekends or holidays. Salary ranges are provided with comprehensive benefits.

Qualifikationen

• Master's degree in Behavioral Health or related field.
• Active, unrestricted independent behavioral health license in your state.
• 3+ years of post-master's clinical experience in hospital/ambulatory settings.
• 3+ years of experience treating mental health and substance use disorders.
• 3+ years of experience utilizing Motivational Interviewing techniques.
• Case Management experience.
• Ability to document in clinical systems while on calls.

Aufgaben

• Conduct comprehensive behavioral health assessments using clinical tools and benefit plan reviews.
• Develop individualized care plans addressing behavioral and physical health needs.
• Coordinate care with providers, community organizations, and support services.
• Apply clinical judgment to identify and mitigate risk factors impacting member outcomes.
• Perform crisis intervention for members and ensure referrals and follow-up.
• Utilize Motivational Interviewing to engage members and promote behavior change.
• Coordinate psychosocial wraparound services with community resources.
• Support cost-effective care through proactive care planning and resource utilization.
