Job summary

CVS Health, a leading health services company, seeks a Licensed Behavioral Health Case Manager for a 100% remote, United States–based role. You will conduct comprehensive assessments, develop integrative care plans, coordinate across providers and community supports, and perform crisis interventions to improve member outcomes.

The position emphasizes Motivational Interviewing, risk mitigation, and virtual documentation, with a Monday–Friday schedule and a competitive benefits package.

Qualifications

• Master's degree in Behavioral Health, Social Work, Counseling, Psychology, or related field.
• Active, unrestricted independent behavioral health license in your state of residence (e.g., LCSW, LISW, LMFT, LPC, LCPC, LPCC, LMHC, or equivalent)
• 3+ years of post-master's direct clinical experience in a hospital, ambulatory, outpatient, or similar clinical setting
• 3+ years of experience treating mental health and substance use disorders
• 3+ years of experience utilizing Motivational Interviewing techniques
• Case Management experience
• Ability to simultaneously talk on the phone and document in clinical systems
• Strong technical proficiency and comfort working in a virtual environment

Responsibilities

• Conduct comprehensive behavioral health assessments using clinical tools, data analysis, and benefit plan reviews
• Develop individualized care plans that address both behavioral and physical health needs
• Coordinate care with providers, community organizations, and support services
• Apply advanced clinical judgment to identify and mitigate risk factors impacting member outcomes
• Perform crisis intervention for members experiencing behavioral health or medical crises and ensure appropriate referrals and follow-up
• Utilize Motivational Interviewing techniques to engage members and promote sustainable behavior change
• Coordinate psychosocial wraparound services and connect members with community resources
• Support cost-effective care through proactive care planning and resource utilization
