Job summary

CVS Health is seeking a licensed Behavioral Health clinician for a 100% remote Clinical Case Manager role. You will conduct assessments and develop care plans to support whole-person health.

Qualifications

• Master's degree in Behavioral Health, Social Work, Counseling, Psychology, or a related field.
• Active, unrestricted independent behavioral health license in your state of residence.
• 3+ years of post-master's direct clinical experience in behavioral health settings.
• 3+ years treating mental health and substance use disorders.
• 3+ years using Motivational Interviewing techniques.

Responsibilities

• Conduct comprehensive behavioral health assessments using clinical tools, data analysis, and benefit plan reviews.
• Develop individualized care plans that address behavioral and physical health needs.
• Coordinate care with providers, community organizations, and support services.
• Apply advanced clinical judgment to identify and mitigate risk factors impacting member outcomes.
• Perform crisis intervention and ensure referrals and follow-up.
• Utilize Motivational Interviewing to engage members and promote behavior change.
• Coordinate psychosocial wraparound services and connect members with community resources.
• Support cost-effective care through proactive planning and resource utilization.
