Job summary

CVS Health Corporation is seeking a licensed Behavioral Health Clinician to serve as a Clinical Case Manager. This fully remote role spans the United States, with a Monday–Friday schedule and no weekends or holidays.

Qualifications

• Master’s degree in Behavioral Health or related field and active, unrestricted license.
• 3+ years of post-master’s direct clinical experience in hospital/ambulatory settings.
• 3+ years of experience treating mental health and substance use disorders.
• 3+ years of experience utilizing Motivational Interviewing techniques.
• Case Management experience.
• Ability to talk on the phone and document in clinical systems.
• Strong technical proficiency and comfort working in a virtual environment.

Responsibilities

• Conduct comprehensive behavioral health assessments using clinical tools and benefit plan reviews.
• Develop individualized care plans addressing behavioral and physical health needs.
• Coordinate care with providers, community organizations, and support services.
• Apply clinical judgment to identify and mitigate risk factors affecting outcomes.
• Perform crisis intervention and ensure appropriate referrals and follow-up.
• Utilize Motivational Interviewing to engage members and promote behavior change.
• Coordinate psychosocial wraparound services and connect with community resources.
• Support cost-effective care through proactive care planning and resource utilization.
