This 100% remote, Monday-Friday role supports whole-person health, with no weekends or holidays, and offers a comprehensive benefits package.

Applicants must hold a master's in a relevant field and an active independent license in their state, with 3+ years of clinical experience.

Qualifikationen

• Master's degree in Behavioral Health, Social Work, Counseling, Psychology, or related field.
• Active, unrestricted independent behavioral health license in your state (e.g., LCSW, LISW, LMFT, LPC, LCPC, LPCC, LMHC).
• 3+ years post-master's direct clinical experience in hospital, ambulatory, outpatient, or similar clinical setting.
• 3+ years of experience treating mental health and substance use disorders.
• 3+ years of experience utilizing Motivational Interviewing techniques.
• Case Management experience.
• Ability to simultaneously talk on the phone and document in clinical systems.

Aufgaben

• Conduct comprehensive behavioral health assessments using clinical tools, data analysis, and benefit plan reviews.
• Develop individualized care plans that address both behavioral and physical health needs.
• Coordinate care with providers, community organizations, and support services.
• Apply advanced clinical judgment to identify and mitigate risk factors impacting member outcomes.
• Perform crisis intervention for members experiencing behavioral health or medical crises and ensure appropriate referrals and follow-up.
• Utilize Motivational Interviewing techniques to engage members and promote sustainable behavior change.
• Coordinate psychosocial wraparound services and connect members with community resources.
• Support cost-effective care through proactive care planning and resource utilization.
