Job summary

CVS Health in the United States is seeking a licensed Behavioral Health Clinical Case Manager to support members with complex mental health and substance use needs. This 100% remote role requires clinical expertise to conduct assessments, develop care plans, and coordinate services, with a focus on whole-person health.

You will work Mon-Fri in a fully remote environment, using Motivational Interviewing and care coordination to improve outcomes while maintaining high safety and quality standards.

Qualifications

• Master's degree in Behavioral Health, Social Work, Counseling, Psychology, or related field.
• Active, unrestricted independent behavioral health license in your state of residence.
• 3+ years of post-master's clinical experience in behavioral health.
• 3+ years of experience treating mental health and substance use disorders.
• Experience using Motivational Interviewing techniques and case management.

Responsibilities

• Conduct comprehensive behavioral health assessments and reviews.
• Develop individualized care plans addressing behavioral and physical health needs.
• Coordinate care with providers, community organizations, and support services.
• Apply clinical judgment to identify and mitigate risk factors.
• Provide crisis intervention and referrals with follow-up.
• Coordinate psychosocial wraparound services and connect to resources.
• Support cost-effective care through proactive planning.
