Traditional Home Health Care Pediatric Division is seeking a Licensed Practical Nurse to join our home-based care team in Stroudsburg, PA. Shifts include both weekday and weekend, with full-time or part-time schedules and a pay rate of $28-$32 per hour.

You will provide one-to-one patient care in home or school settings, administer prescribed medications and therapies, and document the plan of care to ensure quality.

Rate:$28-$32 per hour
Immediate Need:Stroudsburg, PA: Looking for school accompaniment, 8:00am - 3:30 pm

Responsibilities:

• One to One patient care in home/school environment
• Implement and document the plan of care to ensure quality and continuity of care for patient
• Administration of prescribed medication, treatments and therapies
• Patient assessments; coordination of care
• Reports any changes in the client condition to Clinical Manager and physician
• Health promotion, teaching, and training of family members

Available Nursing Shifts:

• Morning, evening or overnight
• 8’s, 10’s, or 12’s (full shift work)
• PRN, Part-Time, or Full-Time
• No weekend or minimum requirements
