Job summary

The Lakes Home Care, Inc. seeks a Licensed Practical Nurse (LPN) to provide skilled nursing care in patients' homes, serving pediatric and/or adult clients with a focus on assessment and individualized care plans.

On each assignment, you will work according to prescribed orders, monitor patient conditions, document data, and collaborate with families and care teams. The Lakes offers flexible hours and competitive pay to support quality home-based care.

Qualifications

• LPN/LVN licensed in the state where the LPN will practice.
• Current TB or Chest X‑Ray.
• Current BLS card.
• One year of professional nursing experience preferred.

• On each assignment, provide skilled nursing care/services in accordance with prescribed orders.
• Assess signs and symptoms indicating physiologic and psychosocial changes in the patient's condition.
• Collect, analyze, and interpret data from health care members and document nursing diagnoses.

• Currently licensed as an LPN/LVN in the state in which the LPN will practice.

• Current TB or Chest X-Ray.

• Current BLS card.

• One year prior professional nursing experience preferred
