Job summary

The Lakes Home Care, Inc. is hiring a Licensed Practical Nurse (LPN) to work with pediatric and/or adult patients in patients' homes.

This role follows the nursing process to assess, plan, implement, and evaluate care while delivering skilled nursing services per prescribed orders. Qualified applicants must hold a current LPN/LVN license in the practicing state, have a current TB test or Chest X-ray, and a valid BLS certification.

Qualifications

• Utilize the nursing process to assess, plan, implement, and evaluate patient care.
• Collects, analyzes, and interprets data and information from health care members and documents actual and/or potential nursing diagnoses.
• On each assignment, provides skilled nursing care/services in accordance with prescribed orders.
• Current TB or Chest X-Ray as a prerequisite for job eligibility.

Responsibilities:

• Utilize the nursing process to assess, plan, implement, and evaluate patient care.

• On each assignment, provides skilled nursing care/services in accordance with prescribed orders

• Assess signs and symptoms indicating physiologic and psychosocial changes in the patient's condition.

• Collects, analyzes, and interprets data and information from health care members and documents actual and/or potential nursing diagnoses.

Qualifications:

• Currently licensed as an LPN/LVN in the state in which the LPN will practice.

• Current TB or Chest X-Ray.

• Current BLS card.

• One year prior professional nursing experience preferred
