Job summary

The Lakes Home Care, Inc. is seeking a Licensed Practical Nurse (LPN) to provide in-home care for pediatric and/or adult patients. You will utilize the nursing process to assess, plan, implement, and evaluate care, provide skilled nursing services per orders, assess signs and symptoms, and document actual or potential nursing diagnoses to support safe, patient-centered home care.

Qualifications

• Currently licensed as an LPN/LVN in the state in which the LPN will practice.
• Current TB or Chest X-Ray.
• Current BLS card.
• One year prior professional nursing experience preferred

• Assess signs and symptoms indicating physiologic and psychosocial changes in the patient's condition.
