Job summary

The Lakes Home Care, Inc. is seeking a Licensed Practical Nurse (LPN) to provide in-home care for pediatric and/or adult patients. You will assess, plan, implement, and evaluate nursing care for assignments while adhering to prescribed orders.

Responsibilities include collecting and analyzing patient data, documenting nursing diagnoses, and ensuring timely, quality nursing services. NY state licensure, BLS, TB or Chest X-ray, and the ability to create your own schedule are preferred.

Qualifications

• Provide skilled nursing care and follow prescribed orders.
• Assess physiologic and psychosocial changes in patients.
• Document nursing diagnoses and care data accurately.

The Lakes Home Care is hiring for a Licensed Practical Nurse (LPN) to work with Pediatric and/or adult patients in their own homes.

Responsibilities:

• Utilize the nursing process to assess, plan, implement, and evaluate patient care.
• On each assignment, provides skilled nursing care/services in accordance with prescribed orders
• Assess signs and symptoms indicating physiologic and psychosocial changes in the patient’s condition.
• Collects, analyzes, and interprets data and information from health care members and documents actual and/or potential nursing diagnoses.

Qualifications:

• Currently licensed as an LPN/LVN in the state in which the LPN will practice.
• Current TB or Chest X-Ray.
• Current BLS card.
• One year prior professional nursing experience preferred
