Job summary

The Lakes Home Care, Inc. is seeking a Licensed Practical Nurse (LPN) to provide skilled nursing care in patients' homes, including pediatric and/or adult populations.

The role uses the nursing process to assess, plan, implement, and evaluate care per orders and to document nursing diagnoses based on data gathered from health care members. Responsibilities include monitoring patient condition, collecting and interpreting data, and delivering high-quality home health services.

Qualifications

• Active LPN/LVN license in the state where the LPN will practice.
• Current TB or Chest X-Ray.
• Current BLS card.
• One year of professional nursing experience preferred.

• Currently licensed as an LPN/LVN in the state in which the LPN will practice.

• Current BLS card.
• One year prior professional nursing experience preferred
