Job summary

The Lakes Home Care is hiring a Licensed Practical Nurse (LPN) to provide in-home nursing care to pediatric and adult patients in Punta Gorda, Florida. Join a compassionate team delivering skilled nursing services in patients' homes with flexible hours.

Responsibilities include assessing, planning, implementing, and evaluating care; documenting findings; and ensuring orders are followed. Required: current LPN license in Florida, BLS, and TB or chest X‑ray; one year of experience preferred.

Qualifications

• Current LPN/LVN license in Florida required.
• Current TB or Chest X-Ray.
• Current BLS card.
• One year of professional nursing experience preferred.

Responsibilities

• Assess, plan, implement, and evaluate patient care.
• Provide skilled nursing services per prescribed orders on each assignment.
• Assess signs and symptoms indicating physiologic and psychosocial changes.

Responsibilities:

• Utilize the nursing process to assess, plan, implement, and evaluate patient care.

• On each assignment, provides skilled nursing care/services in accordance with prescribed orders

• Assess signs and symptoms indicating physiologic and psychosocial changes in the patient’s condition.

• Collects, analyzes, and interprets data and information from health care members and documents actual and/or potential nursing diagnoses.

Qualifications:

• Currently licensed as an LPN/LVN in the state in which the LPN will practice.

• Current TB or Chest X-Ray.

• Current BLS card.

• One year prior professional nursing experience preferred
