Job summary

The Lakes Home Care, Inc. is seeking a Licensed Practical Nurse (LPN) to deliver in-home nursing care for pediatric and adult patients across Florida.

You will work in patients' homes, providing skilled nursing services in accordance with prescribed orders. Your duties include assessing patient needs using the nursing process, developing a care plan, implementing and evaluating care, collecting data, and documenting nursing diagnoses.

Qualifications

• Active LPN/LVN license in Florida.
• TB test or chest X-ray within required period.
• Current BLS (Basic Life Support) certification.
• One year of professional nursing experience preferred.

Responsibilities

• Assess patient condition using the nursing process and document findings.
• Plan, implement, and evaluate patient care per orders.
• Provide skilled nursing interventions in home settings for pediatric and/or adult patients.
• Maintain accurate and timely documentation and communicate with the health care team.

The Lakes Home Care is hiring for a Licensed Practical Nurse (LPN) to work with Pediatric and/or adult patients in their own homes.

Responsibilities:

• Utilize the nursing process to assess, plan, implement, and evaluate patient care.
• On each assignment, provides skilled nursing care/services in accordance with prescribed orders
• Assess signs and symptoms indicating physiologic and psychosocial changes in the patient’s condition.
• Collects, analyzes, and interprets data and information from health care members and documents actual and/or potential nursing diagnoses.

Qualifications:

• Currently licensed as an LPN/LVN in the state in which the LPN will practice.
• Current TB or Chest X-Ray.
• Current BLS card.
• One year prior professional nursing experience preferred
