Job summary

Elite Care Management, Inc. is seeking Skilled Nurses (RN or LPN) to provide one-on-one home health care to patients in their homes. Each shift focuses on one patient, enabling personalized care, with opportunities to learn patient needs and tailor services accordingly.

We offer full-time and part-time roles with shifts such as 6am-6pm on Sundays and PRN on various weekdays and weekends. Competitive pay ranges $35-$40 per hour, with a comprehensive benefits package and weekly pay.

Qualifications

• RN or LPN license required.
• CPR certification required.
• Driver's license required.

Responsibilities

• Observe patient, note significant conditions and reactions, and notify supervisor or physician of changes.
• Administers medications and treatments as prescribed, monitors effectiveness, and documents results.
• Provide one-on-one home health care, align with the patient care plan, and update as needed.
• Demonstrate strong communication skills, both spoken and written.

• Demonstrate understanding organizational and nursing policies and procedures.
• Demonstrate strong communication skills, both spoken and written.
• Demonstrate ability to manage and direct patient care according to the patient care plan and update as needed.
• Administers medications and treatments as prescribed, as well as monitors and documents effectiveness of intervention.
• Intervenes as applicable in response to changes in patient/client condition and reports such changes to the appropriate physician or supervisor.
