Job summary

Water's Edge at Port Jefferson for Rehabilitation and Nursing is seeking a Licensed Practical Nurse (LPN) to provide skilled care on our SNF and Long Term Care units, including medication administration and vital signs monitoring. This role includes a $3,000 sign-on bonus and daily pay options.

Qualifications

• Current LPN license in good standing.
• Experience in skilled nursing or long-term care preferred.
• Must be able to supervise CNAs.

Responsibilities

• Provide care for residents on SNF and Long Term Care units.
• Medication administration, including IVs.
• Take and record vital signs and monitor outcomes.
• Educate residents and families; communicate with physicians and staff.
• Supervise CNAs and coordinate with care team.

• Provide care for residents of our Skilled Unit (SNF) and Long Term Care Unit. Duties Include;
• Medication Administration, including IVs.
• Taking and recording vital signs.
• Providing resident care and treatments with monitoring of outcome.
• Provide resident and family education.
• Communication with physicians, NPs, families and other health care professionals.
