Job summary

Water's Edge at Port Jefferson for Rehabilitation and Nursing seeks a Licensed Practical Nurse (LPN) to join our Skilled Unit and Long Term Care Unit in New York. You will administer medications, including IVs, and monitor vital signs while providing compassionate resident care under RNs and physicians.

Qualifications

• Licensed Practical Nurse (LPN) license currently active.
• Experience in SNF/long-term care preferred.
• Ability to monitor and document resident status and respond to changes.

Responsibilities

• Provide care for residents in Skilled Unit and Long Term Care Unit.
• Administer medications, including IVs, and document administration.
• Take and record vital signs and monitor patient outcomes.
• Provide resident and family education.
• Communicate with physicians, NPs, families and other health care professionals.
• Supervise CNAs and support care team.
