Job summary

The Lakes Home Care, Inc. seeks a dedicated Licensed Practical Nurse (LPN) to provide skilled nursing care at patients' homes, caring for pediatric and/or adult clients as assigned.

You will assess, plan, implement, and evaluate care using the nursing process, document findings, and collaborate with families and health professionals. A Florida LPN license and current BLS are required, with flexible scheduling available.

Qualifications

• LPN/LVN license required in the practicing state.
• TB test or Chest X-Ray is required.
• Current BLS certification.
• One year of professional nursing experience preferred.

• On each assignment, provide skilled nursing care/services per prescribed orders.
• Assess signs and symptoms of physiologic and psychosocial changes in the patient.
• Collect, analyze, and document data and nursing diagnoses.

Responsibilities:

• Utilize the nursing process to assess, plan, implement, and evaluate patient care.
• On each assignment, provides skilled nursing care/services in accordance with prescribed orders
• Assess signs and symptoms indicating physiologic and psychosocial changes in the patient’s condition.
• Collects, analyzes, and interprets data and information from health care members and documents actual and/or potential nursing diagnoses.

Qualifications:

• Currently licensed as an LPN/LVN in the state in which the LPN will practice.
• Current TB or Chest X-Ray.
• Current BLS card.
• One year prior professional nursing experience preferred
