Job summary

St. Joseph Hospice in New Orleans, LA is hiring a full-time Licensed Practical Nurse (LPN) to provide nursing care under the Interdisciplinary Plan of Care.

Candidates must hold a Louisiana LPN license, have 2–3 years of LPN experience, CPR certification, reliable transportation, and hospice experience is preferred. ESOP and company stock are part of the benefits.

Qualifications

• Current Louisiana LPN license required.
• Minimum of 2–3 years as an LPN.
• Current CPR certification required.
• Have dependable transportation, driver’s license, and car insurance.
• Previous hospice experience preferred.

Responsibilities

• Provides nursing and supportive care in accordance with the Interdisciplinary Plan of Care, under the direction of the RN.
• Coordinates with attending physicians and hospice team to ensure optimal patient care.
• Documents all services provided and submits them on appropriate forms.
• Demonstrates knowledge of Medicare/Medicaid hospice regulations.
• Reports changes in patient condition to the RN.

Job duties:

• Provides nursing and supportive care in accordance with the Interdisciplinary Plan of Care, under the direction of the RN.
• Communicates with the attending physicians, other hospice team members, and other agencies as needed to coordinate optimal care and use of community resources for the patient/family
• Document all services provided and submit them on appropriate forms.
• Demonstrates knowledge of Medicare/Medicaid regulations for hospice benefits.
• Report changes in patient condition and needs to RN
