Job summary

St. Joseph Hospice in Lake Charles, LA is hiring for a full-time Licensed Practical Nurse (LPN). You will provide nursing and supportive care under RN supervision, coordinating with physicians and hospice team members to deliver quality care at home or in the facility.

This role requires an LPN license for Louisiana, CPR certification, and 2–3 years of LPN experience with dependable transportation. The position offers an ESOP ownership plan, disability coverage, generous PTO, mileage

Qualifications

• Current Louisiana Licensed Practical Nurse (LPN) license required.
• Minimum of 2-3 years as a Licensed Practical Nurse (LPN).
• Current CPR certification required.
• Have dependable transportation, driver’s license, and car insurance.
• Previous Hospice experience preferred.

Responsibilities

• Provides nursing and supportive care in accordance with the Interdisciplinary Plan of Care, under the direction of the RN.
• Communicates with the attending physicians, hospice team members, and other agencies as needed to coordinate optimal care and use of community resources for the patient/family.
• Document all services provided and submit them on appropriate forms.
• Demonstrates knowledge of Medicare/Medicaid regulations for hospice benefits.
• Report changes in patient condition and needs to RN.
• Mileage reimbursement (adjusted for fuel price changes).
