Job summary

St. Joseph Hospice in Sibley, LA is hiring Part-Time and PRN Licensed Practical Nurses (LPN) to provide nursing and supportive care under the Interdisciplinary Plan of Care. You will coordinate with RNs and hospice team, document services, and communicate with the physician and community resources.

A LA LPN license and CPR are required, with 2–3 years of LPN experience and reliable transportation. Hospice experience preferred.

Qualifications

• Current Louisiana LPN license required
• Minimum of 2-3 years as an LPN
• Current CPR certification required
• Reliable transportation, driver’s license, and car insurance
• Previous Hospice experience preferred

Responsibilities

• Provides nursing and supportive care in accordance with the Interdisciplinary Plan of Care, under the direction of the RN
• Communicates with attending physicians, other hospice team members, and other agencies as needed to coordinate care and use of community resources for patient/family
• Documents all services and submits them on appropriate forms
• Demonstrates knowledge of Medicare/Medicaid regulations for hospice benefits
• Reports changes in patient condition and needs to RN
• ESOP – company-paid ownership benefit
