Job summary

St. Joseph Hospice is hiring a Full-Time Licensed Practical Nurse (LPN) in Covington, LA. You will provide nursing and supportive care under the supervision of an RN, coordinate with physicians and the hospice team, and ensure proper documentation and regulatory compliance.

Qualifications

• Must hold a current Louisiana Licensed Practical Nurse license.
• CPR certification required.
• Reliable transportation and automobile insurance are required.
• Hospice experience is preferred but not mandatory.

Responsibilities

• Provide nursing and supportive care per Interdisciplinary Plan of Care under RN supervision.
• Coordinate care by communicating with attending physicians, hospice team, and community agencies.
• Document all services on appropriate forms and maintain accurate records.
• Demonstrate knowledge of Medicare/Medicaid hospice regulations and eligibility.
• Report changes in patient condition to the RN.
• Support ESOP and company-wide ownership initiatives as part of the team.

Qualifications

• Current Louisiana Licensed Practical Nurse (LPN) license required
• Minimum of 2-3 years as a Licensed Practical Nurse (LPN)
• Current CPR certification required
• Have dependable transportation, driver’s license, and car insurance
• Previous Hospice experience preferred

Job duties:

• Provides nursing and supportive care in accordance with the Interdisciplinary Plan of Care, under the direction of the RN.
• Communicates with the attending physicians, other hospice team members, and other agencies as needed to coordinate optimal care and use of community resources for the patient/family
• Document all services provided and submit them on appropriate forms.
• Demonstrates knowledge of Medicare/Medicaid regulations for hospice benefits.
• Report changes in patient condition and needs to RN
