Job summary

St. Joseph Hospice in Sibley, LA is seeking Part-Time and PRN Licensed Practical Nurses (LPN) to provide nursing and supportive care on a flexible schedule. The position requires an active Louisiana LPN license, CPR, and reliable transportation. Hospice experience is preferred.

Qualifications

• Current Louisiana Licensed Practical Nurse (LPN) license required.

Responsibilities

• Provides nursing and supportive care per the Interdisciplinary Plan of Care under RN direction.
• Communicates with attending physicians and hospice team to coordinate care.
• Documents all services and submits forms accordingly.
• Demonstrates knowledge of Medicare/Medicaid hospice regulations.
• Reports changes in patient condition to RN.
• Supports ESOP program and mileage reimbursement as part of benefits.

Job duties:

• Provides nursing and supportive care in accordance with the Interdisciplinary Plan of Care, under the direction of the RN.
• Communicates with the attending physicians, other hospice team members, and other agencies as needed to coordinate optimal care and use of community resources for the patient/family
• Document all services provided and submit them on appropriate forms.
• Demonstrates knowledge of Medicare/Medicaid regulations for hospice benefits.
• Report changes in patient condition and needs to RN
