Job summary

St. Joseph Hospice in New Orleans, LA, is hiring a Full-Time Licensed Practical Nurse (LPN) to join our hospice care team. You will provide nursing and supportive care under the direction of the RN and coordinate with physicians and other team members to deliver high-quality patient-centered care.

Qualifications

• Current Louisiana Licensed Practical Nurse (LPN) license required
• Minimum of 2-3 years as an LPN
• Current CPR certification required
• Have dependable transportation, driver's license, and car insurance
• Previous Hospice experience preferred

Responsibilities

• Provides nursing and supportive care in accordance with the Interdisciplinary Plan of Care, under the direction of the RN.
• Communicates with the attending physicians, other hospice team members, and other agencies as needed to coordinate optimal care and use of community resources for the patient/family
• Document all services provided and submit them on appropriate forms.
• Demonstrates knowledge of Medicare/Medicaid regulations for hospice benefits.
• Report changes in patient condition and needs to RN
