Job summary

St. Joseph Hospice is hiring full-time Licensed Practical Nurses (LPN) in Baton Rouge, LA. You will provide nursing and supportive care per the Interdisciplinary Plan of Care under the direction of an RN and coordinate with physicians and the hospice team to ensure high-quality patient care.

Qualifications

• Louisiana LPN license is required.
• 2-3 years of LPN experience preferred.
• Current CPR certification required.
• Must have dependable transportation, driver’s license, and car insurance.
• Hospice experience preferred.

Responsibilities

• Provide nursing and supportive care per the Interdisciplinary Plan of Care under RN supervision.
• Communicate with attending physicians and hospice team to coordinate patient care.
• Document all nursing services on appropriate forms.
• Demonstrate knowledge of Medicare/Medicaid regulations for hospice benefits.
• Report changes in patient condition to the RN.

Qualifications

• Current Louisiana Licensed Practical Nurse (LPN) license required
• Minimum of 2-3 years as a Licensed Practical Nurse (LPN)
• Current CPR certification required
• Have dependable transportation, driver’s license, and car insurance
• Previous Hospice experience preferred

Job duties

• Provides nursing and supportive care in accordance with the Interdisciplinary Plan of Care, under the direction of the RN.
• Communicates with the attending physicians, other hospice team members, and other agencies as needed to coordinate optimal care and use of community resources for the patient/family
• Document all services provided and submit them on appropriate forms.
• Demonstrates knowledge of Medicare/Medicaid regulations for hospice benefits.
• Report changes in patient condition and needs to RN
