Job summary

Qualified candidates have a Mississippi LPN license, 2–3 years of LPN experience, CPR certification, and reliable transportation. Hospice experience is preferred, with duties including nursing care under the RN and coordination with the care team.

Qualifications

• Current Mississippi Licensed Practical Nurse (LPN) license required
• Minimum of 2-3 years as a Licensed Practical Nurse (LPN)
• Current CPR certification required
• Have dependable transportation, driver’s license, and car insurance
• Previous Hospice experience preferred

Responsibilities

• Provides nursing and supportive care per the Interdisciplinary Plan of Care under RN direction
• Coordinate with physicians, hospice team, and other agencies for optimal patient care
• Document all services and submit on appropriate forms
• Demonstrates knowledge of Medicare/Medicaid regulations for hospice benefits
• Report changes in patient condition and needs to the RN
• Participates in ESOP and company benefits enrollment
• Provide compassionate, patient-family centered care in a hospice setting
• Maintain professional standards and compliance with policies

Job duties:
