Job summary

St. Joseph Hospice in Covington, LA is hiring a full-time Licensed Practical Nurse (LPN). The role includes providing nursing and supportive care per the care plan under the supervision of an RN, coordinating with physicians and the hospice team, and documenting services accurately.

Mileage reimbursement and ESOP benefits are part of the package. Applicants should hold an active Louisiana LPN license, have CPR certification, and possess reliable transportation.

Qualifications

• Current Louisiana LPN license is required.
• 2-3 years of LPN experience.
• CPR certification required.
• Reliable transportation, driver’s license and car insurance.
• Previous hospice experience preferred.

Responsibilities

• Provides nursing and supportive care per the Interdisciplinary Plan of Care, under the direction of the RN.
• Coordinates care with attending physicians and hospice team to optimize patient outcomes.
• Documents all services provided on appropriate forms.
• Maintains knowledge of Medicare/Medicaid hospice regulations.
• Reports changes in patient condition to the RN.

Qualifications

• Current Louisiana Licensed Practical Nurse (LPN) license required
• Minimum of 2-3 years as a Licensed Practical Nurse (LPN)
• Current CPR certification required
• Have dependable transportation, driver’s license, and car insurance
• Previous Hospice experience preferred

Job duties:

• Provides nursing and supportive care in accordance with the Interdisciplinary Plan of Care, under the direction of the RN.
• Communicates with the attending physicians, other hospice team members, and other agencies as needed to coordinate optimal care and use of community resources for the patient/family
• Document all services provided and submit them on appropriate forms.
• Demonstrates knowledge of Medicare/Medicaid regulations for hospice benefits.
• Report changes in patient condition and needs to RN
