Job summary

Greenbrier Senior Living in Kingston, TN is seeking a PRN Licensed Practical Nurse (LPN) to provide direct resident care in a senior living community. You will follow individualized care plans, administer medications, and document care per policy.

The role requires an LPN license in the state, CPR certification, and at least 1 year of related experience. Physical ability to lift up to 25 pounds and assist with transfers is required.

Qualifications

Responsibilities

  • Provide direct resident care per individualized plan of care.
  • Administer medications with proper documentation and reporting.
  • Perform resident assessments per state standards and facility policy.
  • Observe and report changes in resident condition to supervisor.
  • Document observations, communications, and daily care by end of shift.
  • Maintain and update individualized care plans for each resident.
  • Perform other duties as assigned by supervisor.

What You Will Do:
  • Provide direct resident care services. Follow individualized plan of care for all residents.
  • Administer medications following all standards of practice, state regulations and polices to include all required documentation, reporting and follow up.
  • Perform resident assessments as required by the state standards/ regulations and facility policy, under the direction of the Director of Assisted Living.
  • Observe residents for change in condition and report any changes immediately to supervisor and/or the Director of Assisted Living.
  • Document all pertinent/relevant observations, communication with the resident’s responsible party, clinical procedures and daily assistance/care by the end of each shift.
  • Complete and maintain current individualized care plan for each resident.
  • Other duties as assigned by supervisor.
Supervisory Responsibilities:
  • The position falls under the supervision of the Director of Assisted Living and/or Director of Memory Care but is expected to perform independently and exercise good judgment. The position may assist with supervision, direction, and reviews of the work of other associates and may participates in performance appraisals and disciplinary action.

  • On a daily basis, throughout the workday: Must be capable of sitting, standing, walking, climbing stairs, reaching, turning, bending, stooping, crouching and kneeling, reaching over the head, grasping with both hands, and fine manipulation. Must physically be able to lift up to 25 pounds unassisted. Must physically be able to carry, transfer, push and pull, and reposition residents with assistance or using appropriate equipment.
