Job summary

Crescent Wood Senior Living is seeking a compassionate Licensed Practical Nurse (LPN) to join our team in Titusville, FL. This role offers a competitive wage of $29.00–$31.00 per hour plus a $3,000 sign-on bonus and comprehensive benefits.

You will provide direct nursing care to residents in Assisted Living and Memory Care, administer medications, and support a compassionate, resident-centered team. Qualifications include a Florida LPN license, CPR certification, and at least 1 year of nursing

Qualifications

• Current Florida LPN license in good standing.
• Current CPR certification.
• High school diploma or GED required.
• At least 1 year of nursing experience preferred (senior living/long-term care a plus).

Responsibilities

• Provide direct nursing care to Assisted Living and Memory Care residents.
• Administer medications and treatments per physician orders and policies.
• Complete resident assessments under supervisor guidance.
• Observe residents and communicate changes promptly to supervisors.
• Document medications, treatments, assessments, and observations accurately.
• Maintain and update individualized resident care plans.
• Collaborate with caregivers, physicians, and leaders to deliver resident-centered care.
• Communicate professionally with residents, families, physicians, and staff.
• Mentor and support caregivers and nursing staff as needed.
• Promote dignity, independence, comfort and quality of life.

What You'll Do

• Provide direct nursing care to Assisted Living and Memory Care residents.
• Administer medications and treatments in accordance with physician orders, state regulations, and community policies.
• Complete resident assessments under the direction of the Director of Assisted Living.
• Observe residents for changes in condition and promptly communicate concerns to the appropriate supervisor.
• Accurately document medications, treatments, assessments, observations, and resident care before the end of each shift.
• Maintain and update individualized resident care plans.
• Collaborate with caregivers, medication aides, physicians, and department leaders to deliver exceptional resident-centered care.
• Communicate professionally with residents, families, physicians, and fellow team members.
• Assist in mentoring and supporting caregivers and other nursing staff as needed.
• Promote resident dignity, independence, comfort, and quality of life.
• Perform other duties as assigned.

What We’re Looking For

• Current Florida Licensed Practical Nurse (LPN) license in good standing.
• Current CPR certification.
• High school diploma or GED required.
• At least 1 year of nursing experience preferred (senior living, long-term care, rehabilitation, or skilled nursing experience is a plus).
• Strong assessment, communication, and documentation skills.
• Compassionate, dependable, and team-oriented.
• Ability to multitask in a fast-paced environment while maintaining exceptional resident care.
• Ability to meet the physical requirements of the position, including standing, walking, bending, lifting up to 25 pounds unassisted, and assisting residents with transfers using proper equipment.
