Job summary

A senior living community in Kentucky is seeking a Lead Licensed Practical Nurse (LPN) to provide direct resident care and ensure adherence to care plans. The ideal candidate will support a positive environment for residents, administer medications, and oversee nursing staff. Candidates should possess at least 2 years of supervisory experience in a healthcare setting, a valid LPN license, and CPR certification. Qualifications

  • Must have at least 2 years of related work experience in a leadership capacity.
  • Experience in senior living, health care, or hospitality preferred.
  • Must be capable of physical tasks throughout the workday.
  • At least 2 years of related work experience in a leadership or supervisory capacity is required.
  • Senior living, health care or hospitality experience is preferred.
  • Must be physically capable of sitting/standing for long periods and lifting up to 25 pounds regularly.

Responsibilities

  • Provide direct resident care services according to individualized plans.
  • Administer medications and perform resident assessments as required.
  • Document observations and communicate with responsible parties.
  • Observe changes in resident condition and report promptly to supervisor.
  • Document observations and communication with residents and families.
  • Maintain current individualized care plans for each resident.
  • Attend interdisciplinary resident care meetings.
  • Record vital signs, weights, and glucose as assigned.
  • Assist with activities of daily living and resident mobility.

What You Will Do:

  • Provide direct resident care services. Follow individualized plan of care for all residents.
  • Administer medications following all standards of practice, state regulations and polices to include all required documentation, reporting and follow up.
  • Perform resident assessments as required by the state standards/ regulations and facility policy, under the direction of the Director of Assisted Living.
  • Observe residents for change in condition and report any changes immediately to supervisor and/or the Director of Assisted Living.
  • Document all pertinent/relevant observations, communication with the resident’s responsible party, clinical procedures and daily assistance/care by the end of each shift.
  • Complete and maintain current individualized care plan for each resident.
  • Attend all scheduled interdisciplinary resident care meetings.
  • Check and record vital signs, weights, and blood sugars as assigned.
  • Provide for or assist residents with activities of daily living as needed, including bathing/showing according to schedule, oral care, toileting, dressing, etc. Encourage residents to participate in scheduled facility activities providing assistance in getting to activities as needed. Assist in the planning and implementation of resident activities as needed.

  • On Call Rotation
  • Other duties as assigned by supervisor.

Supervisory Responsibilities:

  • The position falls under the direct supervision of the Director of Assisted Living and is expected to perform independently and exercise good judgment. The position assists with supervision, direction, and reviews of the work of other associates and may participates in performance appraisals and disciplinary action.

The ideal candidate will have the following:

  • High school diploma or equivalent (GED) required.
  • Licensed Practical Nurse in applicable state required.
  • Current CPR certification required.
  • At least 2 years of related work experience in a leadership or supervisory capacity required
  • Senior living, health care or hospitality experience preferred.
  • On a daily basis, throughout the workday: Must be capable of sitting, standing, walking, climbing stairs, reaching, turning, bending, stooping, crouching and kneeling, reaching over the head, grasping with both hands, fine manipulation, and lifting up to 25 pounds regularly unassisted.
