Job summary

The Hamlet Rehabilitation and Healthcare Center at Nesconset is seeking an experienced Licensed Practical Nurse (LPN) to join our skilled nursing and long-term care units. You will administer medications, monitor vital signs, and provide comprehensive resident care in a collaborative team environment.

We offer a $3,000 sign-on bonus and daily pay options, with recognition meals and free uniforms. A current LPN license is required to join our dedicated team serving residents and families with

Qualifications

• Must hold a valid LPN license.
• Experience providing care in skilled nursing or long-term care settings preferred.
• Ability to administer medications and monitor residents’ conditions.

Responsibilities

• Provide care for residents in Skilled and Long Term Care units.
• Administer medications, including IVs where applicable.
• Take and record vital signs consistently.
• Provide resident care and monitor outcomes of treatments.
• Educate residents and families about care plans.
• Communicate with physicians, NPs, families, and other professionals.
• Supervise CNAs as assigned.
