Job summary

Premise Health Inc. is hiring a Full Time Licensed Practical Nurse (LPN) for our Health Center in Countryside, IL.

You will provide direct patient care to employees and dependents, administering medications, wound care, and specimen collection under provider supervision. The role requires an LPN license in the state, an associate degree or equivalent, BLS certification, and at least three years of ambulatory/occupational health or ED experience.

Qualifications

• Licensed as a Practical or Vocational Nurse in the state of practice
• Associate's degree or equivalent from a two‑year college or technical school
• Current hands-on Basic Life Support (BLS) certification required
• At least three years of clinical experience as LPN/LVN in ambulatory, occupational health, or ED setting

Responsibilities

• Provide direct patient care under supervision of healthcare providers
• Administer medications, perform wound care, and collect specimens per orders
• Maintain equipment, ensure a safe, infection‑controlled environment
• Maintain accurate patient health records
• Perform additional duties as assigned

As a Full Time Licensed Practical Nurse (LPN), you'll provide care to client employees and their dependents in our Health Center located in Countryside, IL.

Schedule:

• Mon, Wed, Thu, Fri 8:30 am - 5:00 pm; Tue 10:00 am - 7:00 pm
• Will be required to work one Sat approx. every 6 weeks 6:30 am - 12:00 pm

What You'll Do

• Provides direct patient care and collaboration under the supervision of healthcare providers, and within scope of practice
• Performs and documents medical tasks to include medication administration wound care, observes reactions to treatments, and specimen collection such as urine, blood, and sputum based on provider orders
• Maintains medical equipment, ensures a clean and safe environment, and follows infection control procedures
• Maintains patient health records to ensure accurate and up-to-date records
• Performs other duties as assigned

What You'll Bring

• Graduate of National League for Nursing accredited program, Associate's degree (A.S.) and/or equivalent from two-year college or technical school
• Currently Licensed as a Practical or Vocational Nurse in state of practice required
• Current certification with hands-on training in AHA, ARC or equivalent Basic Life Support for health care providers is required
• At least three years of clinical experience as LPN/LVN in an ambulatory care, occupational health, or emergency department setting
