Job summary

Premise Health is seeking a Full Time Licensed Practical Nurse (LPN) to deliver direct patient care at our Health Center in Bend, Oregon. The role follows a 4-10 hour shift pattern with rotations and a weekend requirement, supported by a comprehensive benefits package.

The ideal candidate will hold an active LPN license in Oregon, be AED/ARC/BLS certified, and bring at least 3 years of ambulatory or occupational health experience.

Qualifications

• LPN/LVN license in state of practice required.
• Current Basic Life Support certification (AHA/ARC) required.
• Minimum 3 years of clinical experience in ambulatory/occupational health/emergency dept.

Responsibilities

• Provide direct patient care under supervision of healthcare providers.
• Perform and document medical tasks including medication administration and wound care.
• Maintain medical equipment and environment; adhere to infection control procedures.
• Maintain accurate and up-to-date patient health records.
• Perform other duties as assigned.

As a Full Time Licensed Practical Nurse (LPN), you'll provide care to client employees and their dependents in our Health Center located in , . The scheduled hours are 4-10 hour shifts, Monday, Wednesday, Friday 6:30am-5:00pm, Tuesday and Thursday 7:30am-6:00pm. Rotating one Saturday per month 8:00am-1:00pm.

What You'll Do

• Provides direct patient care and collaboration under the supervision of healthcare providers, and within scope of practice
• Performs and documents medical tasks to include medication administration wound care, observes reactions to treatments, and specimen collection such as urine, blood, and sputum based on provider orders
• Maintains medical equipment, ensures a clean and safe environment, and follows infection control procedures
• Maintains patient health records to ensure accurate and up-to-date records
• Performs other duties as assigned

What You'll Bring

• Graduate of National League for Nursing accredited program, Associate's degree (A.S.) and/or equivalent from two-year college or technical school
• Currently Licensed as a Practical or Vocational Nurse in state of practice required
• Current certification with hands-on training in AHA, ARC or equivalent Basic Life Support for health care providers is required
• At least three years of clinical experience as LPN/LVN in an ambulatory care, occupational health, or emergency department setting
