Job summary

Community Care Physicians’ Albany Family Medicine is seeking a Full Time LPN to join our team in Albany, NY. This role is a 40-hour week with hours from 8:00am to 4:30pm and offers a $3,500 recruitment/retention bonus. The position includes vaccines administration, patient documentation, and support for patient flow.

Compensation ranges from $26.00 to $33.00 per hour, with a comprehensive benefits package and room for growth. Prior EMR experience and NYS LPN license are required.

Qualifications

• Valid NYS LPN license.
• Previous LPN experience in a medical practice or as Medical Assistant/Patient Care Tech desirable.
• Strong computer skills with EMR experience.
• Knowledge of basic nursing principles, infection control, patient confidentiality.
• Ability to maintain confidential information and accurate charting.

Responsibilities

• Maintain timely patient flow.
• Prepare patients for visits per office requirements.
• Obtain specimens (urine, blood, etc.) as directed by the provider.

• Perform clinical office procedures as directed (PFT, EKG, nebulizer, meds, immunizations).
• Process patient prescriptions.
• Discuss test results with patients per provider instructions.

Responsibilities

• Administer and record vaccines.
• Document verbal conversations with patients when appropriate.
• Document information into patient record in an accurate, legible and timely manner.
• Maintain timely patient flow.
• Prepare patient for visit as appropriate, based on office/provider requirements.
• Obtain specimens (i.e., urine, blood, etc.) from patients as directed by the provider.
• Accurate and timely order entry into the lab system.
• Perform clinical office procedures as directed by the provider (i.e. PFT, EKG, nebulizer, medication administration and immunizations)
• Process patient prescriptions.
• Discuss test results with patients per provider's instructions.

Compensation: $26.00 - $33.00 per hour
