Community Care Physicians is seeking LPNs for per diem, part-time, and full-time roles in New York. Flexible schedules based on availability and office needs.

Multiple office locations including Albany and Latham, with diverse clinical responsibilities and a supportive, team-oriented environment. Responsibilities include vaccines administration, patient documentation, specimen collection, lab order entry, and assisting with various office procedures.

Formación

• Valid NYS LPN license required.
• Experience with EMR systems.
• Knowledge of basic nursing principles and protocols.

Responsabilidades

• Administer and record vaccines.
• Document verbal conversations with patients when appropriate.
• Document information into patient records accurately and timely.
• Maintain timely patient flow.
• Prepare patients for visits according to office/provider requirements.
• Obtain specimens (urine, blood, etc.) as directed by the provider.
• Enter orders accurately into the lab system.
• Perform clinical office procedures as directed (PFT, EKG, nebulizer, immunizations).
• Process patient prescriptions.
• Discuss test results with patients per provider instructions.

Community Care Physicians is looking for per diem, part-time, and full-timeLPNs to join their team!

Office locations include:

• Albany Family Medicine
• Charlton Family Medicine
• Family Practice Ballston Spa
• Latham Medical Group
• General Surgery
• Family Practice Slingerlands
• Schodack Internal Medicine and Pediatrics
• CapitalCare Pediatrics Guilderland
• Community Care Pediatrics Saratoga

Responsibilities:

• Administer and record vaccines.
• Document verbal conversations with patients when appropriate.
• Document information into patient record in an accurate, legible and timely manner.
• Maintain timely patient flow.
• Prepare patient for visit as appropriate, based on office/provider requirements.
• Obtain specimens (i.e., urine, blood, etc.) from patients as directed by the provider.
• Accurate and timely order entry into the lab system.
• Perform clinical office procedures as directed by the provider (i.e. PFT, EKG, nebulizer, medication administration and immunizations)
• Process patient prescriptions.
• Discuss test results with patients per provider's instructions.

Compensation: $26.00 - $33.00 per hour

CCP salary ranges are designed to be competitive with room for professional and financial growth. Individual compensation is based on several factors unique to each candidate, such as work experience, qualifications, and skills. Some roles may also be eligible for overtime pay.

Requirements

• Valid NYS LPN license.
• Previous LPN experience in a medical practice or prior experience as a Medical Assistant or Patient Care Tech.
• Strong computer skills with experience using EMR.
• Knowledge of basic nursing principles and protocols.
• Knowledge of policies and procedures related to infection control, environmental safety, and patient confidentiality.
• Maintain strictest confidentiality and discretion.
• Knowledge of medical chart reporting/records.
• Skill in appropriate assessment and assistance techniques.
