Community Care Physicians is seeking a full-time LPN for their Family Practice Great Oaks office in Albany, NY. This 40-hour week role offers a competitive hourly pay and a $3,500 recruitment/retention bonus.

Responsibilities include immunizations, patient prep, specimen collection, EMR documentation, and patient communication. Qualifications include a valid NYS LPN license and prior medical practice experience; strong EMR skills and a commitment to confidentiality are essential.

Qualifications

• NYS LPN license is required.
• Previous LPN experience in a medical practice or prior experience as a Medical Assistant or Patient Care Tech.
• Strong computer skills with experience using EMR.
• Knowledge of basic nursing principles and protocols.
• Knowledge of policies and procedures related to infection control, environmental safety, and patient confidentiality.
• Maintain strictest confidentiality and discretion.
• Knowledge of medical chart reporting/records.
• Skill in appropriate assessment and assistance techniques.

• Accurate and timely order entry into the lab system.
• Perform clinical office procedures as directed by the provider (i.e. PFT, EKG, nebulizer, medication administration and immunizations)
• Process patient prescriptions.
• Discuss test results with patients per provider's instructions.

Our Family Practice Great Oaksoffice is looking for a full-time LPN to join their team!

This is a 40 hour week. The hours are: Monday-Friday 8:00am-4:30pm or 8:30am-5:00pm with rotating Saturdays 8:00am-1:00pm.

**This position is eligible for up to a $3,500 recruitment and retention bonus!!**

Responsibilities:

• Administer and record vaccines.
• Document verbal conversations with patients when appropriate.
• Document information into patient record in an accurate, legible and timely manner.
• Maintain timely patient flow.
• Prepare patient for visit as appropriate, based on office/provider requirements.

Compensation: $26.00 - $33.00 per hour

CCP salary ranges are designed to be competitive with room for professional and financial growth. Individual compensation is based on several factors unique to each candidate, such as work experience, qualifications, and skills. Some roles may also be eligible for overtime pay.
