Job summary

Community Care Physicians invites applications for a Full Time LPN at our Family Medicine office in Clifton Park, NY. This is a 40-hour-per-week position supporting vaccine administration, patient flow, and entry of medical data into electronic records.

The role requires a valid NYS LPN license and prior medical office experience. Candidates should be comfortable with EMR systems and performing routine clinical tasks under provider direction.

Qualifications

• Valid NYS LPN license.
• Experience as LPN in a medical practice or as a Medical Assistant/Patient Care Tech.
• Strong computer skills with EMR experience.
• Knowledge of basic nursing principles, infection control, and patient confidentiality.

Responsibilities

• Administer and record vaccines.
• Document verbal conversations with patients when appropriate.
• Enter patient information accurately into the chart.
• Maintain timely patient flow throughout the visit.
• Prepare patients for visits as required by office/providers.
• Obtain specimens (urine, blood, etc.) as directed by the provider.
• Enter orders accurately into the lab system.
• Perform clinical procedures (PFT, EKG, nebulizer, meds, immunizations).
• Process patient prescriptions.
• Discuss test results with patients per provider instructions.

Job description

Our Community Care Family Medicine office is looking for a Full Time LPN to join their team!

This is a 40 hour per week position.

Responsibilities

• Administer and record vaccines.
• Document verbal conversations with patients when appropriate.
• Document information into patient record in an accurate, legible and timely manner.
• Maintain timely patient flow.
• Prepare patient for visit as appropriate, based on office/provider requirements.
• Obtain specimens (i.e., urine, blood, etc.) from patients as directed by the provider.
• Accurate and timely order entry into the lab system.
• Perform clinical office procedures as directed by the provider (i.e. PFT, EKG, nebulizer, medication administration and immunizations)
• Process patient prescriptions.
• Discuss test results with patients per provider's instructions.

Compensation: $26.00 - $33.00 per hour
