Job summary

Community Care Physicians operates a Schodack Internal Medicine and Pediatrics office seeking a full-time LPN to join the team. This 40-hour weekly role runs Monday through Friday from 7:30am to 4:30pm, with a $3,500 recruitment and retention bonus for eligible candidates.

You will administer vaccines, document conversations, maintain patient flow, and support clinical procedures under the provider's direction, ensuring accurate charting and timely care.

Qualifications

• Valid NYS LPN license.
• Experience as LPN in medical practice or as Medical Assistant/Patient Care Tech.
• Strong computer skills and EMR experience.
• Knowledge of nursing principles and infection control procedures.
• Ability to maintain confidentiality and handle patient records.

Responsibilities

Responsibilities

• Administer and record vaccines.
• Document verbal conversations with patients when appropriate.
• Document information into patient record in an accurate, legible and timely manner.
• Maintain timely patient flow.
• Prepare patient for visit as appropriate, based on office/provider requirements.
• Obtain specimens (i.e. urine, blood, etc.) from patients as directed by the provider.
• Accurate and timely order entry into the lab system.
• Perform clinical office procedures as directed by the provider (i.e. PFT, EKG, nebulizer, medication administration and immunizations).
• Process patient prescriptions.
• Discuss test results with patients per provider's instructions.

Compensation

$26.00 - $33.00 per hour
