Job summary

Community Care Physicians is seeking a full-time Licensed Practical Nurse for Family Practice Great Oaks in New York. The position offers 40 hours per week with standard weekday hours and rotating Saturdays.

The role includes administering vaccines, charting conversations, maintaining patient flow, and performing select clinical procedures under provider instruction. Compensation ranges from $26.00 to $33.00 per hour with a comprehensive benefits package.

Qualifications

• Valid NYS LPN license.
• Previous LPN experience in a medical practice or hospital setting recommended.
• Strong computer skills with EMR software experience.
• Knowledge of basic nursing principles and infection control procedures.

Responsibilities

• Administer and record vaccines.
• Document verbal conversations with patients when appropriate.
• Document information into patient records accurately and timely.
• Maintain timely patient flow.
• Prepare patients for visits according to office/provider requirements.
• Obtain specimens from patients as directed by the provider.
• Enter orders into the lab system accurately.
• Perform clinical office procedures (e.g., PFT, EKG, nebulizer) as directed.
• Process patient prescriptions and discuss test results with patients.

Job description

Our Family Practice Great Oaksoffice is looking for a full-time LPN to join their team!

This is a 40 hour week. The hours are: Monday-Friday 8:00am-4:30pm or 8:30am-5:00pm with rotating Saturdays 8:00am-1:00pm.

**This position is eligible for up to a $3,500 recruitment and retention bonus!!**

Responsibilities:

• Administer and record vaccines.
• Document verbal conversations with patients when appropriate.
• Document information into patient record in an accurate, legible and timely manner.
• Maintain timely patient flow.
• Prepare patient for visit as appropriate, based on office/provider requirements.
• Obtain specimens (i.e., urine, blood, etc.) from patients as directed by the provider.
• Accurate and timely order entry into the lab system.
• Perform clinical office procedures as directed by the provider (i.e. PFT, EKG, nebulizer, medication administration and immunizations)
• Process patient prescriptions.
• Discuss test results with patients per provider's instructions.

Compensation: $26.00 - $33.00 per hour

Requirements

• Strong computer skills with experience using EMR.
• Knowledge of basic nursing principles and protocols.
• Knowledge of policies and procedures related to infection control, environmental safety, and patient confidentiality.
• Maintain strictest confidentiality and discretion.
• Knowledge of medical chart reporting/records.
• Skill in appropriate assessment and assistance techniques.
