Job summary

Traditional Home Health Care Pediatric Division is seeking a compassionate Licensed Practical Nurse (LPN) to join our team providing one-to-one care in patients’ homes and schools across Northumberland County, PA. You will implement the plan of care, administer medications, and coordinate patient care with clinical staff.

We offer flexible shifts, excellent pay, and a supportive team with 24/7 clinical assistance.

Qualifications

• Licensed Practical Nurse (LPN) role in home health and school environments.
• Pediatric and adult patients cared for in home settings.
• Flexible scheduling available.

Responsibilities

• One to One patient care in home/school environment
• Implement and document the plan of care to ensure quality and continuity of care for patient
• Administration of prescribed medication, treatments and therapies
• Patient assessments; coordination of care
• Reports any changes in the client condition to Clinical Manager and physician
• Health promotion, teaching, and training of family members

Sunbury, PA: Monday - Friday, school accompaniment, 8:00am - 3:00pm/ShikellamySchool District
Sunbury, PA: Trach/Vent overnight shift available
Dewart, PA: Wednesday 7:00am - 6:00pm/Tuesday, Thursday, Friday, Saturday & Sunday 6:00pm - 11:00pm
