Job summary

Traditional Home Health Care Pediatric Division is seeking a Licensed Practical Nurse (LPN) to join our team providing in-home, one-on-one care for pediatric and adult patients in the Honesdale region. We offer flexible shifts, including weekday and weekend options, with full-time and part-time schedules available.

Qualifications

• Must hold an active LPN license in PA.
• Experience in pediatric or home health care preferred.
• Ability to provide one-on-one in-home care.

Responsibilities

• One to One patient care in home/school environment
• Implement and document the plan of care to ensure quality and continuity of care for patient
• Administration of prescribed medication, treatments and therapies
• Patient assessments; coordination of care
• Reports any changes in the client condition to Clinical Manager and physician
• Health promotion, teaching, and training of family members
