Job summary

Traditional Home Health Care Pediatric Division in Berwick, PA seeks a compassionate Licensed Practical Nurse (LPN) to provide one-on-one in-home care for pediatric and adult patients. Flexible weekday and weekend shifts, with full-time or part-time schedules, support your work-life balance and professional growth.

The LPN will administer medications, implement the care plan, and coordinate with the clinical team to ensure high-quality, safe care in patients' homes.

Qualifications

• Licensed Practical Nurse (LPN) in good standing.
• Ability to provide one-on-one care in home settings.
• Flexibility for weekday and weekend shifts, full- or part-time.

Responsibilities

• One to One patient care in home/school environment
• Implement and document the plan of care to ensure quality and continuity of care for patient
• Administration of prescribed medication, treatments and therapies
• Patient assessments; coordination of care
• Reports any changes in the client condition to Clinical Manager and physician

Rate: $28-$32 per hour
Immediate Need: Berwick, PA: 7am-3pm/3pm-11pm flexible shifts available
