Traditional Home Health Care Pediatric Division is seeking a compassionate Licensed Practical Nurse (LPN) to provide one-to-one care in patients' homes and school settings in Danville, PA. The role offers weekday, weekend, full-time and part-time shifts with competitive pay and weekly direct-deposit payroll.

We offer flexible scheduling, paid training, and a supportive team with 24/7 clinical support. Pediatric and adult skilled nursing services are provided in home environments.

Qualifications

• LPN license required.
• Home health experience preferred.

Responsibilities

• One-to-one patient care in home/school environment.
• Implement and document the plan of care to ensure quality and continuity of care for patient.
• Administration of prescribed medications, treatments and therapies.
• Patient assessments; coordination of care.
• Reports any changes in the client condition to Clinical Manager and physician.
• Health promotion, teaching, and training of family members.
