Job summary

The Lakes Home Care, Inc. is seeking a licensed practical nurse (LPN) to provide skilled nursing care to pediatric and/or adult patients in their homes.

You will use the nursing process to assess, plan, implement, and evaluate care, following prescribed orders. Responsibilities include on each assignment delivering nursing services, assessing changes in patient condition, collecting and documenting data, and communicating with health care teams.

Qualifications

• Active LPN/LVN license in the state of practice.
• TB or Chest X-Ray current.
• Current BLS card.
• One year of professional nursing experience preferred.

• On each assignment, provides skilled nursing care/services in accordance with prescribed orders
• Assess signs and symptoms indicating physiologic and psychosocial changes in the patient’s condition.
• Collects, analyzes, and interprets data and information from health care members and documents actual and/or potential nursing diagnoses.

Qualifications:

• Currently licensed as an LPN/LVN in the state in which the LPN will practice.
• Current TB or Chest X-Ray.
• Current BLS card.
• One year prior professional nursing experience preferred
