Angels Care Home Health in Flagstaff, AZ seeks a Licensed Practical Nurse (LPN) for full-time work. The position offers a $2,5000 Sign-On Bonus and comprehensive benefits as described by the employer.

The LPN will administer skilled nursing care under RN supervision, coordinate care with families and the interdisciplinary team, and support patients in their home environment. A valid LPN license, driver’s license, and CPR/Hepatitis records are required.

Qualifications

• Graduate of an accredited practical/vocational nursing program.
• Current state LPN license.
• Valid Driver's License.
• Two years’ LPN experience; home health preferred.
• CPR certification and Hepatitis profile.

Responsabilités

About the position:

The primary function of the LPN is to administer skilled nursing care, under the supervision of a registered nurse, for clients of all ages in their place of residence. Coordinates care with the interdisciplinary team, patient/family, and referring agency.

Qualifications:

• Graduate of an accredited school of practical/vocational nursing.
• Current state license as an LPN.
• Current State’s Driver’s License.
• Two years’ experience as an LPN, home health preferred.
• Proof of current CPR and Hepatitis profile.
