Job summary

Angels Care Home Health is seeking a Licensed Practical Nurse (LPN) to deliver skilled nursing care in patients' homes under the supervision of a registered nurse. You will coordinate care with the interdisciplinary team, patient and family, and referring agencies.

Qualifications

• Graduate of an accredited school of practical/vocational nursing.
• Current state license as an LPN.
• Current State’s Driver’s License.
• Two years’ experience as an LPN, home health preferred.
• Proof of current CPR and Hepatitis profile.
