Angels Care Home Health is seeking a Licensed Practical Nurse (LPN) to provide skilled nursing care in clients' homes under the supervision of a registered nurse. You will coordinate with the care team, patients, and families to ensure continuity of care.

The ideal candidate has an accredited LPN program background, an active LPN license, CPR certification, and 2+ years of LPN experience (home health preferred). A valid driver’s license is required.

Qualificações

• Graduate of an accredited school of practical nursing (LPN).
• Current state license as an LPN.
• Current state driver’s license.
• Two years’ experience as an LPN; home health preferred.
• Proof of CPR certification and Hepatitis profile.

Responsabilidades

• Administer skilled nursing care under supervision of a registered nurse for clients in their homes. Coordinate care with the interdisciplinary team, patient/family, and referring agency.

About the position:

The primary function of the LPN is to administer skilled nursing care, under the supervision of a registered nurse, for clients of all ages in their place of residence. Coordinates care with the interdisciplinary team, patient/family, and referring agency.

Qualifications:

• Graduate of an accredited school of practical/vocational nursing.
• Current state license as an LPN.
• Current State’s Driver’s License.
• Two years’ experience as an LPN, home health preferred.
• Proof of current CPR and Hepatitis profile.
