Job summary

Angels Care Home Health is seeking a Licensed Practical Nurse (LPN) to provide skilled nursing care in patients' homes under the supervision of a registered nurse. You will coordinate with the interdisciplinary team, patients, and families to deliver high-quality home health services.

Qualified candidates will have an LPN license, CPR certification, a valid driver’s license, and at least two years of LPN experience, preferably in home health.

Qualifications

• Graduate of an accredited school of practical/vocational nursing.
• Current state license as an LPN.
• Current State’s Driver’s License.
• Two years’ experience as an LPN, home health preferred.
• Proof of current CPR and Hepatitis profile.

Responsibilities

• Administer skilled nursing care under the supervision of a registered nurse in clients' homes.
• Coordinate care with the interdisciplinary team, patients, and families.

About the position:

The primary function of the LPN is to administer skilled nursing care, under the supervision of a registered nurse, for clients of all ages in their place of residence. Coordinates care with the interdisciplinary team, patient/family, and referring agency.
