Job summary

Amedisys, one of the largest home health and hospice providers in the U.S., is seeking a licensed practical nurse to deliver in‑home care in Florida. You will verify plans of care with the RN before visits, collaborate with physicians and nurses, and teach patients and caregivers self‑care techniques.

You’ll check vital signs, reconcile medications, prepare treatment supplies, and document care promptly. Amedisys offers competitive pay, benefits, and opportunities for growth.

Qualifications

• One year of clinical LPN experience.
• Current LPN license for Florida.
• Current CPR certification.
• Valid driver's license with reliable transportation and liability insurance.

Responsibilities

• Verify plan of care with the RN prior to each visit.
• Collaborate with physician and RN frequently to ensure patient needs are being met; Notify them as soon as possible when patient condition changes.
• Teach patients and caregivers self-care techniques.
• Check vital signs.
• Review all medications and perform a medication reconciliation at each visit.
• Perform specialized procedures ordered by physician (i.e. foley catheter change).
• Prepare equipment and materials for treatments using the aseptic technique.
• Complete documentation timely and accurately.
