Job summary

Amedisys is seeking a licensed practical nurse to join our home health team in Oregon. You will verify plans of care with the RN before each visit, collaborate with physicians and RNs, teach patients self-care, monitor vital signs, reconcile medications, and perform physician-ordered procedures in patients' homes, ensuring accurate documentation.

• LPN with 1 year of clinical experience.
• Current LPN license in the state(s) you are assigned to work.
• Current CPR certification.
• Valid driver's license with reliable transportation.

Responsibilities

• Verify plan of care with the RN prior to visits.
• Collaborate with physician and RN to ensure patient needs are met.
• Teach patients and caregivers self-care techniques.
• Check vital signs.
• Review medications and perform a medication reconciliation at each visit.
• Perform specialized procedures ordered by physician (e.g. foley catheter change).
• Prepare equipment and materials for treatments using aseptic technique.
• Complete documentation timely and accurately.
